Nutritional Medicine Initial Exam & Treatments
Your first Nutritional Medicine appointment begins with a review of your detailed health history and metabolic assessment, followed by a head-to-toe physical examination. Please bring copies of previous imaging and lab reports as they may provide valuable information about your past and current state of health.
If warranted, lab and diagnostic testing may be performed at your initial appointment or scheduled for a later date (additional charges apply). Based on testing recommendations, a follow-up appointment will be scheduled within 1-2 weeks to review findings.

NUTRITIONAL MEDICINe Appointment | 30 Min
30 minutes is generally the BEST OPTION for most Nutritional Medicine patients for follow-up care. This appointment is for ESTABLISHED patients to review how at-home treatments are going, discuss progress or set-backs, address questions & concerns, and establish on-going strategies. NUTRITIONAL MEDICINE appointment | 60 MIN
This appointment is for ESTABLISHED Nutritional Medicine patients that require extended treatment time, or with new/exacerbated conditions. For ESTABLISHED Nutritional Medicine patients only who require follow up laboratory evaluation to assess or continue care. Please be on time for your lab draw appointment.Fees for labs vary depending on what is being tested; the base cost of the lab draw appointment is $12. BOOK ONLINE

30 or 60 minute Follow up appointments for ESTABLISHED patients can be scheduled as telemedicine appointments (INITIAL Nutritional Medicine appointment must be done in-person). The doctor will send an emailed link for a HIPAA secured platform to complete the appointment virtually. You will be required to have a phone or computer with internet access to complete this session. Payment due at time of service.

If the doctor has requested you FAST for your labs (typically for cholesterol assessment) this means NO food or beverage aside from water for 10-12 hours.

Sage no longer contracts with medical insurance companies, however, Guest’s requesting to utilize your medical benefits will receive a detailed bill of services to submit to your insurance company, flex spending or health savings account for potential reimbursement directly back to you.

Depending on your presentation, a radiological imaging referral may be necessary for further diagnosing.
If your condition(s) are beyond the capabilities of this office, your practitioner will assist you with a referral to the appropriate physician.
